TRANSFERS
The New York-Presbyterian Physician Access Transfer Center is a 24/7 streamlined patient transfer service for physicians who wish to transfer patients to NYPH from another hospital.

| To arrange for the transfer of a patient, call the NewYork-Presbyterian Hospital Physician Access Transfer Center at 800-NYP-STAT. |
| The following information requested must be initiated by the patient's Attending Physician of Record: |
- Patient's name, date of birth and weight.
- Patient's primary diagnosis/required service.
- Requested NYPH campus: Columbia, Morgan Stanley Children's Hospital or Weill Cornell/Komansky Center for Children's Health.
- Name and phone number of sending physician.
- Name of sending hospital and phone number of nursing unit.
- Patient's status:
- Current condition
- Desired time for transfer
- Plan of care
- Fax patient demographic information to NYP STAT.
